+ Can I fill out my New Patient Forms before I come?
Yes! Download our intake forms here and fill them out any time before you visit. E-mail them to us at lsdg.team@gmail.com or or print them out and bring them with you. For more patient forms click here.
+ Do you accept insurance?
We participate with many insurance plans including Aetna PPO, Cigna Open Access, Cigna PPO, Delta Dental Premier, Guardian PPO and Metlife PDP. We will happily verify your insurance before you arrive. You will need to provide us with your current insurance information along with the primary account holder's birthday. To find out if we accept your plan, please call our office at 516-437-0249 or submit your information using this form.

We accept Mastercard, Visa, American Express and Discover credit cards, personal checks as well as cash. We also accept Care Credit and Apple Pay. To pay your bill click here.
